This Privacy Policy applies to the activities of ePhysio Pty Ltd (ACN 119 977 543).

ePhysio recognises that your privacy is very important to you and that you have a right to control your personal information. Providing personal information is an act of trust and we take that seriously. ePhysio is committed to complying with high standards of privacy protection. Unless you directly give us consent to do otherwise, ePhysio will only collect and use your personal information as set out below.

What information we collect

If you are just browsing our website:

Information you provide directly by using ePhysio will be collected. When you subscribe as an ePhysio user:

When you become an ePhysio subscriber, we collect personal information from you directly. ePhysio collects this information only with your consent.

Information collected may include: name, address, telephone number, email address, date of birth, occupation, previous injury and illness history, referral source, current injury details, and applicable purchasing information including credit card credit card number and expiration date.

We may also collect your personal information from your practitioner regarding your injury and illness history and current injury details. We will only collect this information with your consent. Such information will only be used in accordance with this Policy. We will also invite you to complete a Subjective Feedback Form which is a subjective self assessment form completed by you.

On completion of the Subjective Feedback Form:

The provision of this information is required in order to subscribe to our Programs and to enable us to provide the services (including hosting and operating the website).

We will also collect information from your for the purposes of compiling your "My Exercise Room" page on the website. This part of the website will contain your Subjective Feedback Form, details of your current subscription, and your usage of the Program. This information may be viewed by you, ePhysio, your practitioner or as otherwise set out in this privacy policy.

We will also collect information to assist us to monitor your progress through the Programs.



Once you have become a subscriber, we keep a record your usage of ePhysio. We may ask you to complete a feedback questionnaire regarding your use of the website. Information received from feedback questionnaires on the website will be sent with your consent to your referrer registered with ePhysio and kept by ePhysio to enhance the ePhysio Programs. Except as otherwise provided in this Policy, this information will not be disclosed to any other companies or organizations. This is only to help us gain feedback to enhance the Programs.

Any collection of other personal information not explained above will only occur with consent from you.

Storage & security of your details ePhysio will endeavor to take all reasonable measures to keep information secure that we hold about you and to keep your information up to date and accurate. Personal information is stored on ePhysio for a maximum of six months, if it is not renewed by you starting a new subscription. The employees at ePhysio are legally obliged to respect the confidentiality of any personal information held by ePhysio. ePhysio will not be responsible for any damages you, or any third party, may suffer as a result of any transmission of personal information that you make to this website through the internet, or that you expressly or implicitly authorise the website to make, or for any errors or any changes made to any such transmitted information, other than changes which we made, or errors which are caused by our negligence.

Please note also that ePhysio may use overseas facilities to process or back up its information. As a result, we may transfer your personal information to our overseas facilities for storage. However, this does not change any of our commitments to safeguard your privacy.

Our file of information will be made available to you within 14 days.

Should you wish to change personal information that may have changed, is inaccurate or out of date, you can do this by simply by changing your information by entering "My Exercise Room" page then selecting Update Account Details page or by emailing us at info@ephysio.com. If you wish to remove your personal information from the ePhysio database at any time you can do so via entering "My Exercise Room" page then selecting Update Account Details page or you can email us at info@ephysio.com and we will take all reasonable steps to delete it unless we need to keep it for legal reasons.

ePhysio provides optional links to visit websites outside of the ePhysio portal. ePhysio does not have any control over these websites and we are not responsible for the information provided or the conduct of companies linked to our website.
