This Privacy Policy applies to the activities of ePhysio Pty Ltd (ACN 119 977 543).

ePhysio recognises that your privacy is very important to you and that you have a right to control your personal information. Providing personal information is an act of trust and we take that seriously. ePhysio is committed to complying with high standards of privacy protection. Unless you directly give us consent to do otherwise, ePhysio will only collect and use your personal information as set out below.

If you are just browsing our website:

Information you provide directly by using ePhysio will be collected. When you subscribe as an ePhysio user:

When you become an ePhysio subscriber, we collect personal information from you directly. ePhysio collects this information only with your consent.

Information collected may include: name, address, telephone number, email address, date of birth, occupation, previous injury and illness history, referral source, current injury details, and applicable purchasing information including credit card credit card number and expiration date.

We may also collect your personal information from your practitioner regarding your injury and illness history and current injury details. We will only collect this information with your consent. Such information will only be used in accordance with this Policy. We will also invite you to complete a Subjective Feedback Form which is a subjective self assessment form completed by you.

On completion of the Subjective Feedback Form:

(i) you can submit the form directly to your practitioner via email on the website; or

(ii) you can complete the Subjective Feedback Form for self monitoring of your progress. In this case it will not be emailed directly to your practitioner, but your practitioner will have direct access to view this form on website.

When you visit our website, our server attaches a small data file called a "cookie" to your computer's memory for record keeping purposes.

ePhysio uses cookies to help enhance your experience on the website by remembering information that you have previously given to us. They can make it easier for you to record information and help us to ensure that information provided to you is most relevant to your needs. Cookies themselves do not contain personal information about you; we do not store any information that identifies you inside cookies.

We also use cookies to provide us with aggregate (anonymous) information on how people use our website and to help us to know what they find useful on our website to continually improve the design of our website. We do not link this information back to your identity or other information that you have provided to us.

• provide you with other information about ePhysio services and programs and other matters which may interest you;

• for those of you with an email address, send you an email newsletter; and/or

• provide you with relevant information regarding additional products and services to assist your rehabilitation needs.

For the purposes of viewing our website you will be supplied with a user name and password. You are responsible for the security of your password and log-in information. You take responsibility to ensure the confidentiality of this information.

Please endeavour to keep confidential any passwords and account details used on ePhysio. This is an important role in keeping your personal information secure. Please notify us immediately if there is any unauthorised use of your account by any other internet user of any other breach of security.

Your trust in ePhysio is very important to us. If at any time you would like to know exactly what personal information we are holding at any particular time, you can view via your "My Exercise Room" page and Update Account Details page or you can email us at info@ephysio.com. Our file of information will be made available to you within 14 days.

Should you wish to change personal information that may have changed, is inaccurate or out of date, you can do this by simply by changing your information by entering "My Exercise Room" page then selecting Update Account Details page or by emailing us at info@ephysio.com. If you wish to remove your personal information from the ePhysio database at any time you can do so via entering "My Exercise Room" page then selecting Update Account Details page or you can email us at info@ephysio.com and we will take all reasonable steps to delete it unless we need to keep it for legal reasons.

ePhysio provides optional links to visit websites outside of the ePhysio portal. ePhysio does not have any control over these websites and we are not responsible for the information provided or the conduct of companies linked to our website. No endorsement is made, whether express or implied, by the presence of a link or the presence of other third party details or information on this website.

The ePhysio website (www.ephysio.com.au) uses Liquid Vision to host and monitor the website. This enables us to collect and monitor statistics for research and marketing purposes and to help us enhance our service to you. Any information collected will remain anonymous and confidential.

This is ePhysio’s current Privacy Policy. ePhysio may from time to time review and revise the Privacy Policy. ePhysio reserves the right to change its Privacy Policy at any time and notify you by posting the updated Privacy Policy on the ePhysio website www.ephysio.com.au. The amended Privacy Policy will apply between us whether or not we have given you specific notice of any change.
